Download Meeting Attendance Record Sheet (PDF for printing, to help you keep track and stay accountable) This form is traditionally used by such diversion and treatment programs as accountability and proof of attendance, and most A.A. meetings in the local area are both familiar with and willing to fill out and sign the document for each meeting he attends.
For online meeting attendance records, reach out to the chairperson of the meeting (leads the meeting for that day) either via chat and/or by arriving early before the meeting begins and asking the chairperson directly. They will coordinate with you to update your meeting attendance records from there. Often the chairperson will provide this via email, that you can forward to whoever is requesting your meeting attendance records, or the chairperson may email that individual directly on your behalf.
If you weren’t able to coordinate with the chair of the meeting that day, attend that same meeting again and ask who chaired the meeting on the day you need attendance records for, and they can help coordinate that with you.
